What to bring for your first visit
- Details of any medications being taken.
- Details of any medical conditions.
- Dental Insurance Information (if any). This would include the name of the insurance carrier, the group/plan/policy number and I.D./certificate/SIN number.
- Referral and X-rays from your dentist (if they haven’t already been sent).
If you need to take antibiotics prior to dental appointments, please make sure you have done so.
